FAQ
Yes, if you or your loved one have an active Medicare Part B plan, our mobile outpatient OT and PT services are covered. Most patients on Medicare plans with a secondary insurance can expect to pay nothing for each therapy appointment.
It's not as straightforward as knowing your insurance provider's name. Coverage for services can vary, ranging from full coverage to requiring a co-payment, depending on your specific insurance plan. Please contact us with the patient's name, date of birth, and member ID (insurance number), and we will provide a complimentary verification of insurance benefits and coverage for our in-home outpatient therapy services. If needed, we will also request authorization from your insurance company for these services.
Medicare beneficiaries can go directly to physical or occupational therapists without a referral or visit to a physician. Once the therapy evaluation is complete, it will be sent to your physician. For other insurances, please call us to verify if your plan requires a doctor's order for therapy services.
No. You don’t need to be home bound to receive at-home physical or occupational therapy. AgeWell Rehab Services is a MOBILE outpatient practice. Services are covered under "outpatient", but our therapists come to your home.
AgeWell Rehab Services is located in Connecticut, currently serving Fairfield County and New Haven County. There is no physical practice. In all the neighborhoods we serve, we come to you to make the therapy experience as convenient as possible.
Because our therapists perform in-home physical and occupational therapy, they don’t have the overhead of large health centers and are able to spend more time focused on you during your appointment. In fact, AgeWell Rehab therapists spend up to 4x more time with each patient than they could in a traditional clinic. You’ll get a full 53-60 minute appointment that’s 100% focused on you and your care plan.
Our physical therapists are state-licensed and board certified by the American Physical Therapy Association (APTA). Our occupational therapists are state-licensed and board certified by the National Board for Certification in Occupational Therapy (NBCOT). Both physical therapists and occupational therapists are mandated by professional licensing boards and organizations to participate in continued education to ensure that healthcare professionals stay updated on the latest advancements, research, and best practices in their respective fields. Continued education helps maintain and improve the quality of care provided by PTs and OTs, keeping them informed about new techniques, technologies, and evidence-based approaches to better serve their patients.
Please find the REFERRAL FORM at the top of the page and fill out the information. We will be in touch with the patient as soon as possible. For physician offices, you may fax your own referral form to (475) 474-6132. Thank you!
